Prednisone 60 mg/m2 po daily, days 1–7

Cycles 2, 4, 6:

Ifosfamide 800 mg/m2 IV daily, days 1–5

Mesna 200 mg/m2 IV daily, 0, 4, and 8 h after ifosfamide

Methotrexate 150 mg/m2 IV for 30 min then 1.35 g/m2 IV for 23.5 h, with leucovorin rescue

Vincristine 2 mg IV day 1

Cytarabine 150 mg/m2/day by continuous infusion on days 4 and 5

Etoposide 80 mg/m2 IV over 1 hr on days 4 and 5

Dexamethasone 10 mg/m2 orally days 1 through 5

Intrathecal methotrexate 15 mg, cytarabine 40 mg, and hydrocortisone 50 mg on days 1 and 5
